For your first appointment

Please arrive at least 15 minutes before your appointment. New pateints will meet with a Patient Services Representative to register.

Please bring the following registration documents:

  • A photo ID
  • Your insurance card or letter documenting coverage
  • Proof of income

Scheduled and missed appointments

We will call you the night before your appointment. Please press 1 to confirm your appointment or press 3 to cancel it. You must cancel your appointment at least 4 hours in advance, or the appointment will be marked as a missed appointment. If you miss several appointments, we will only be able to offer you walk-in services.

If you are more than 10 minutes late to your appointment, you may lose your appointment.

Payment and Prepayment Options

We accept cash, check, Visa, or MasterCard as payment options for all of CUHCC services.

Annual proof of family income

All patients will be asked to provide proof of household income. CUHCC needs this information because it receives federal assistance to provide its services. Please bring one of the following documents to prove your income:

  • A current check stub
  • A copy of income tax return
  • Retirement benefits statement
  • Workers compensation or unemployment benefits statement
  • Disability or social security benefits statement
  • A letter from employer, if paid in cash

What is CUHCC?
We are a community clinic that welcomes all patients of every race, color, ethnicity, language, and belief. We have been serving our community for over 50 years.

Can I walk in?
Yes, come in! We are accepting new patients. Walk-in service is available for medical, dental and mental health care between 8:00 a.m. and 5:00 p.m. Monday through Friday.

Do you have pediatric doctors or pregnancy care?
Yes, we have great pediatric and pregnancy care providers! We also have dental and mental health services for children, men, and women.
