At the Community-University Health Care Center (CUHCC), we are transforming urban health care and educating the next generation of healthcare professionals. We partner with areas across the University of Minnesota to provide an array of educational opportunities.
The Social Work Triage Specialist addresses same-day, episodic, psychosocial needs. The Specialist connects CUHCC patients to community resources and CUHCC programs. This position is filled with paid student positions and Social Work interns (MSW supervision on site).

Minnesota Northstar GWEP
Minnesota Northstar Geriatrics Workforce Enhancement Program (GWEP) is a federally-funded workforce training award, using evidence to promote best practices and training for those who care for geriatric patients. CUHCC is collaborating with Northstar GWEP to transform medical education and primary care to be age- friendly. You can learn more about GWEP here.
